Facilitator Tip #7:  Make the analysis visual
Because a root cause analysis has multiple causes the simplest and most effective way to capture the information is visually.  During the analysis the facilitator should ensure that all causes are written down and visible.  This visual approach can be captured on paper, chart paper, dry-erase board or electronically using an LCD projector.  All of the problems and causes that people offer during an investigation will be captured within specific cause and effect relationships that link together creating the incident.  Evidence that validates a particular cause can be added directly to the bottom of that cause.  Documenting an investigation by starting with the goals and then mapping the cause-and-effect relationships creates a visual dialogue that improves the way individuals see and process the incident.  This visual analysis naturally creates a more constructive, objective exchange making the facilitator’s role significantly easier and more productive.  Because the cause-and-effect relationships are visual it also becomes the record of the investigation at the same time.
